高度恶性子宫内膜间质肉瘤高度恶性子宫内膜间质肉瘤 肿瘤来自内膜间质,宫底部生长,形成柔软的息肉状或分叶状的肿块向宫腔突出。肿瘤切面灰色或黄色,呈鱼肉状,局部有出血、坏死及囊性变,早期即可有肌层及血管浸润,累及或转移至浆膜面及盆腔。镜下有两种形态。一为均质型,可见间质细胞极为丰富,腺体少见。细胞胞质少,有大核,核分裂象多于10个/10个高倍视野。这种均质型的高度恶性子宫内膜间质肉瘤有的也可形成性索样的结构。另一种为多形型,肿瘤细胞具有多种形态,自梭形至多边形,核异型性大,核大小、形状、及染色质密度均不一致,且有大而奇形怪状的细胞核。高度恶性的间质肉瘤恶性程度高,预后差,大部分患者3年内死于盆腔转移或血行转移至肝、肺等处。处理原则同子宫肉瘤。
