肾素-血管紧张素-醛固酮系统    肾素-血管紧张素-醛固酮系统  十肽  根据肾素-血管紧张素-醛固酮三者之间的密切关系提出的一个新概念。其作用首先为肾素把血浆中的血管紧张素原(在α2球蛋白中)水解生成血管紧张素Ⅰ(10肽)它能刺激肾上腺髓质使之释放肾上腺素。但对血管的收缩作用较弱。血液和组织中,特别是肺组织中有转换酶,这种酶可使血管紧张素Ⅰ降解,生成血管紧张素Ⅱ(8肽)。后者可进一步被氨基肽酶水解为血管紧张素Ⅲ(7肽)。血管紧张素Ⅱ是一种生物活性很高的升压物质,并可使肾上腺皮质增加醛固酮的合成和分泌醛固酮,又可促进肾小管对钠的重吸收,具有保钠和保水的作用,使细胞外液量增加,血管紧张素Ⅲ也有缩血管和促进醛固酮合成和分泌的作用,但血中的浓度低。所以血管紧张素Ⅱ的作用是主要的,因此三者之间密切联系,不仅有生理理论意义,而且也用于临床解释某些水钠潴留现象,例如肝硬化,出现水肿和脱水后,常伴有继发性醛固酮增多症,其机制则与本系统有关。
