变质甘蔗中毒 食变质甘蔗发生的真菌毒素中毒,从1972年起在中国河北、天津、河南、辽宁、内蒙古、陕西、江苏、贵州和山东等13个省、市、自治区屡有发生,共计发病178起,822人,死亡77人。甘蔗于秋季自广东、广西和福建运到北方保存过冬,若保存不当,甘蔗可变质,次年1~4月份出售时,食之引起中毒。多为散发病例。主要临床表现为中枢神经系统受损,严重者1~3日内死亡,重症者可遗留下终生残疾。 变质甘蔗中毒发病急,潜伏期最短者只有10多分钟,也有长达17小时者。重症病人和死亡者多为儿童。症状最初为呕吐,头晕,进而出现抽搐,四肢强直、屈曲、内旋,手呈鸡爪状,斜视,双下肢痉挛性瘫痪等。 病原体为甘蔗节菱孢(Arthrinium sacchari)、蔗生节菱孢(A.saccharicola)和暗孢节菱孢(A.phaeosperi-num)。在中毒甘蔗样品中,节菱孢为优势菌之一,其检出率为58.7~65.6%;在中毒流行区市售变质甘蔗中检出率为14.8%;正常样品中一般不能检出。毒素成分为3-硝基丙酸 (O2N-CH2-CH2-CO2H)。 变质甘蔗外皮失去应有的光泽,质软,瓤部呈浅褐或深褐色,有或无霉点,嗅之无异味或有霉味和酒糟味。纵切成薄片直接镜检,甘蔗细胞内有大量霉菌菌丝侵染。 根据明确的食用霉变甘蔗史,发病后的临床症状,在排除急性脑炎、脑膜炎、脑外伤及其他食物或化学物质中毒的可能后,即可作出变质甘蔗中毒的诊断。临床检验血、尿、便正常,肝肾功能正常,脑电图显示弥散性病变。后遗症病人以锥体外系受损为主要临床表现,脑的CT检查双侧豆状核毁坏性病变为其规律性改变。目前还缺乏特效疗法。急性期应积极控制脑水肿,改善脑血液循环,这对预防后遗症的发生有重要意义。为预防变质甘蔗中毒要加强食品卫生工作,宣传有关知识。各级卫生防疫机构要抽查贮存的甘蔗,严禁变质的甘蔗上市。商业从业人员要加强贮存甘蔗的管理。
