义齿义齿denture代替缺失的天然牙的人造牙。又称假牙。重度牙周病、龋齿及外伤等原因,可造成牙列部分甚至全部缺失。牙齿缺失后可影响面容、发音、咀嚼功能,增加肠胃负担,若不及时修复还可出现邻牙倾斜移位,对颌牙伸长,使咬合错乱。牙列部分缺失,可用固定义齿或可摘义齿修复,全无牙时可用全口义齿(总义齿)修复。通常用固定义齿修复切牙或1~2个后牙,不论缺牙多少均可用可摘义齿修复。固定义齿完全由基牙负担,即在基牙上作冠或嵌体为桥基,缺牙区作桥体,整体粘固在基牙上,故基牙必须健壮。可摘义齿可有几种形式。一般在余牙上放金属卡环,使义齿固位不脱落,由基托放在缺牙区齿槽粘膜上;也有用栓道附着体固位的形式;还有一种叫改良式双重冠可摘义齿,先将基牙用冠修复,并在其上戴上有双重冠的可摘义齿,故基牙也要较健壮或需经过增强治疗(如牙体部分切除、联冠等),使力量分散(多用些基牙,多增加基托面积)。做固定义齿时需将基牙磨除较多,才能使修复体与基牙密合,不致造成继发龋。可摘义齿只需很少磨牙,尤其是称为分段义齿的可摘义齿,充分利用基牙倒凹及软组织倒凹进行固位。但对余牙的牙冠形态也要作一定的修整,以消除不利的力,避免咬腮咬舌等。固定义齿体积小,异物感少,患者容易适应,咀嚼效率高。可摘义齿异物感较明显,要经过适应阶段,并要注意饭后摘下清洗。缺牙越多,义齿体积越大,支持的牙齿越少,咀嚼效率越低。故牙齿不要轻易拔除,对病牙应采取措施加以保存。牙齿保存越多,义齿承力可以分散,咀嚼效能就恢复较好,且可较持久。全无牙以后,唇部皱瘪,颊部塌陷,咀嚼功能丧失,患者镶牙的要求迫切,可用人造牙列固着在塑料基托上的总义齿来修复,总义齿的固位主要靠两种力量:一是吸附力,唾液和基托内面与无牙颌表面粘膜间形成异分子间的引力(如两块玻璃间有水时可相吸住)。二是负压,当基托密贴在无牙颌表面,四周封闭良好时,二者间产生小于大气压力的负压,义齿表面的大气压力使之不脱落。总义齿的承力则主要靠齿槽,贴合与承托的面积愈大,固位和承力的效果才越好。一般上前牙以外观为主,最好恢复接近原来的外形。后牙以咀嚼功能为主,可改变人工牙的形态,如选用低尖或无尖牙,但要增加牙齿的锐度和耐磨度,以利咀嚼,并必须注意正中、前伸和侧方平衡,这有利于义齿的稳定,也可减少齿槽的萎缩。拔牙后镶牙,一般要在伤口愈合后进行。伤口愈合良好为时约需2~3个月,至少1个半月,这样病人就有一段时间无牙。有一种方法是在拔牙前取模,作好义齿,拔牙后立即戴入,叫预成义齿。这样就免得有缺牙的时期。病人对义齿较易习惯,颌间关系也较易恢复。但这只适应于前牙或留一对上下颌有咬合关系的前磨牙。留牙太多义齿不易密合。一定时间后还得对预成义齿加以增填修改,以适合伤口愈合后的情况。
