贝克认知治疗贝克认知治疗Beck's cognitive therapy L!连1寸乞一左寻;。 ‘徐俊觅撰粱会西宙、贝克认知治疗(Beek’5 eognitivetherapy)由当代最著名的抑郁症认知治疗研究者贝克(Beek，A.T.，1976)所创。贝克研究抑郁症时发现，抑郁症病人存在几种形式的认知歪曲。他设想抑郁症至少部分是对客观经验的一种过分的、消极的曲解所致。抑郁症病人常见的认知歪曲有选择性概括(选择了细节而忽略了明显的矛盾证据)、武断推论(没有证据便作出结论)、过度普遍化(将一个偶然事件中取得的信念不恰当地用于其他情况)以及夸大(对不良事件的意义作出过度估计)等。认知治疗用很多方法帮助病人认识这些认知歪曲引起的消极的自动性思想(negative automaticthoughts)，例如:“假如我不能达到完美程度，就不会有人爱我”，就是自动性思想的例子。认知治疗训练让接受治疗的病人区分事实和信念，将信念、假设和预断作为“假说”进行严格的检验。医师与病人在认识问题、设计检验信念的试验和实施治疗计划时主动协作。贝克在认知治疗中也采用了一系列行为技术，但重点在于改变病人的内在体验，改变其信念和感觉而不是外部行为。治疗每周1次，共15~25次。开始时应告诉病人认知治疗如何起作用，然后给予家庭作业帮助他们评估现有的功能状况。第二次，根据病人特殊的经验，说明思考对情绪和行为的关系。以后告诉病人如何认识、监测和记录消极的自动性思想与有关的情绪及情境，设计对自动性思想的合理反应。最后讨论这些自动性思想及其认知基础。因此，贝克的认知疗法是一种认知行为治疗方法，系统地运用经验过程来认识、评估和改变适应不良的信念系统。同传统心理治疗的不同在于，医师在治疗中是主动的、指导性的，着重于“这里和现在”而不是过去。研究证明，认知治疗对减轻中度和重度单相抑郁症症状有良好效果。有研究报告说，认知治疗的效果略高于抗抑郁剂的疗效(R ush等报告，中度或重度抑郁症完全缓解率达83%)。目前，贝克的认知治疗方法正逐步扩大其应用范围，如焦虑性障碍与进食障碍，并取得一些进展，
