高渗性非酮症糖尿病昏迷高渗性非酮症糖尿病昏迷 *糖尿病急性代谢紊乱的一个临床类型。好发于50～70岁老年患者。常在感染、急性胃肠炎、胰腺炎、脑血管意外。严重肾脏疾患、血液透析治疗以及某些药物如糖皮质激素、免疫抑制剂、噻嗪类利尿剂等诱因作用下发生。表现为先有多尿、多饮、恶心、呕吐、失水逐渐加重，细胞内脱水严重，出现神经精神症状，如嗜睡、幻觉、定向障碍、偏盲、上肢拍击样粗震颤、癫痫样抽搐等，最后陷入昏迷，甚至休克。实验室检查为高血糖，高血浆渗透压，多数伴有高血钠和氮质血症，血糖高至33.3mmol／L(600mg／dl）以上，血钠达150mmol／L或更高，本病病死率较高，故应早期诊断和治疗。一般用小剂量胰岛素静脉滴注治疗，同时应积极补液，注意补充钾盐，并要注意防止脑水肿的发生，积极治疗诱发因素和并发症。
