心肌病心肌病myocardialdiseases原因不明的心肌疾病。1980年世界卫生组织将心肌病分为3类:扩张型心肌病、肥厚型心肌病及限制型心肌病。特异性心肌病指原因明确的心肌病或继发于已知疾病的心脏损害。①扩张型心肌病。病因不明,可能与遗传、感染、营养、中毒及免疫等多种因素有关。组织学表现为心肌细胞变性肥大,肌原纤维减少及胶原组织增多。发病早期可无临床症状,心脏正常,此后心脏渐扩大,并出现心功能不全及各种类型的心律失常,常有附加心音、奔马律及杂音,有时可伴体或肺循环血栓栓塞。心电图ST-T改变,甚至可出现病理性Q波及各种心律失常,X线心扩大及肺瘀血,超声心动图全心扩大,室壁运动减弱。诊断需除外继发心肌病。治疗上积极控制心功能不全、致命性心律失常,心脏移植术可改善预后。②肥厚性心肌病。病因不明,有遗传倾向,可为不对称室间隔肥厚、普遍心肌肥厚或心尖心肌肥厚。心腔变心,心肌细胞肥大,排列紊乱,结缔组织增生。临床出现呼吸困难、心胶痛、晕厥、猝死,晚期可有心力衰竭。胸骨左缘可闻粗糙心缩期喷射性杂音。X线不同程度心扩大。心电图显示左室肥厚、异常Q波及心律失常。超声心动图室间隔肥厚,与左室游离壁厚度之比>1.3及SAM氏征。可采用β-阻滞剂或钙拮抗剂异搏定减轻症状,慎用硝酸甘油及洋地黄。③限制型心肌病。最少见,主要是心内膜增厚,表现为充血性心力衰竭,心脏不大或仅轻度增大。类似缩窄性心包炎。主要是对症治疗。
