菠萝过敏 食菠萝或吸入菠萝蛋白酶后发生的过敏现象。致病因素为菠萝所含的菠萝蛋白酶,对特异性体质者可引起过敏反应。常于幼年发病。该酶可用于食品工业及制药工业。工作中吸入该酶可表现发作性呼气性呼吸困难,伴哮鸣,持续数分钟至数小时。菠萝蛋白酶是一种可溶性抗原特异性成分,通过淋巴细胞呈递至特异的IgE型浆细胞,刺激IgE抗体(反应素)时产生的抗体附着于肥大细胞,当菠萝蛋白酶再进入机体时,作为抗原分子与致敏的肥大细胞或嗜碱性粒细胞膜上的两个 IgE搭桥结合,释出组胺(H)、嗜酸性粒细胞趋化因子(ECF-A)、嗜中性粒细胞趋化因子(NCF)、过敏性慢反应物质(SRS-A)、血小板聚集因子(PAF)、 前列腺素(PG)、缓激肽等生物活性物质,引起支气管平滑肌收缩和粘膜分泌增加,导致哮喘。该病的治疗可选用支气管舒张药控制急性发作。菠萝中含一种苷类,食菠萝后可刺激口腔粘膜,致发痒、发麻。菠萝蛋白酶作为异体蛋白质,可致剧烈腹痛、呕吐、腹泻、皮肤瘙痒、荨麻疹、呼吸困难、脉细弱、紫绀、血压下降等。有特异体质者应避免与过敏原接触以预防发病。民间经验食菠萝前用盐水浸泡可破坏菠萝蛋白酶。菠萝煮熟后食用,菠萝蛋白酶亦可遭破坏。
