先天愚型先天愚型Down's syndrome,mongolism 又称“21-三体综合征”。1846年Seguin首先描述。1866年英国医生Down再次报道后称为Down综合征。1959年法国Lejeune等证实此综合征病因为一个G组染色体(后来确定为21号),故又称为21-三体综合征。本病核型有三种:①21-三体型,即47,+21,其染色体总数为47,多一条第21号染色体。多数是由于母亲形成卵子时,第21号染色体不分离,与正常精子受精后所致。出生患儿的母亲年龄多大于35岁。②易位型:常见的是14/21易位,患儿的核型为46,-14+t(14q;21q),染色体总数仍为46条,少了一条14号染色体,多了一条由14号和21号染色体长臂所形成的易位染色体,但仍多一条21号长臂部分。患儿母亲年龄多在30岁以下,再生子女发生本病的可能性较大,但染色体易位总的来说出现率较低。③嵌合型:核型为46/47+21。有两个细胞系,一为细胞核型正常,另一为21-三体。是因受精卵早期分裂时发生21号不分离形成。患儿发育迟滞,头小,脸扁平,鼻短,耳小,舌大而厚且经常外伸,肢体短小,第5指短且向内弯,睑裂向外上倾斜,眼间距较宽,常有内眦赘皮,虹膜发育不全,智力低下,先天性心脏畸形,肝脏脂肪变性等。治疗为控制反复发作的呼吸道感染和治疗先天性心脏病。本病患者50%死于1岁以内,存活者平均寿命30岁左右。女性可以生育,男性不育。
