冠脉循环 营养心脏本身的血管系(即冠脉系统)中的血液循环。冠脉系统的动脉为左右冠状动脉及其分支,它们运送血液营养心肌细胞。血液流过毛细血管和静脉以后返回右心房。冠状动脉是主动脉的第一对分支,它的血压较高,血流速度较快,循环路径短,所以冠脉的血液供应相当充分。冠脉循环的正常运转,保证了心脏能不停地进行泵血。 冠状动脉各分支之间有侧支吻合,但人类正常的冠脉侧支较细小,血流量很小。因此一旦冠状动脉的某一分支突然阻塞,就会造成心肌缺血,从而导致严重后果。如果冠脉阻塞是缓慢形成的,则侧支可逐渐扩张,建立起新的侧支循环以代偿之。由于冠状动脉功能性改变或器质性病变引起的冠状血流和心肌需求之间不平衡而导致的心肌损害称为冠心病。中国最常见的冠状动脉病变是冠状动脉粥样硬化。该病如果是心肌暂时性缺血,会引起心绞痛;如果是冠状动脉被凝血块堵塞,或发生神经性痉挛,血液不能流通,这就是心肌梗塞。严重时危及生命。冠心病自古就存在,中国马王堆汉墓女尸是世界上发现的经现代医学证明患冠心病的最早病例。近20多年来冠心病在西文国家已成为流行病,此病多见于中老年人。中国各地区普查表明,40岁以上冠心病的发病率为4~7%。冠心病的防治已为全世界所关注。
