免疫病理学 研究免疫系统功能异常（或继发性异常）和免疫应答所引起的病理现象的科学。它是免疫学的分支学科之一，涉及范围较广，包括超敏反应（变态反应）、自身免疫、免疫增生和免疫缺陷等。免疫系统识别和排斥抗原性异物虽然在多数情况下对机体是有利的，但是，也有一些免疫反应不利于机体，能引起功能紊乱或（和）组织损害，例如对移植物的排斥和对自身成分的免疫应答。 主要内容 免疫增生 当免疫活性细胞过度增生时，就出现原发性的病理过程，即免疫增生性疾病。正常的（反应性的）免疫增生可以自我限制，经过一段病程得到康复。恶性免疫增生，如何杰金氏病、巨球蛋白血症、重链病、多发性骨髓瘤等。多发性骨髓瘤是一种以浆细胞大量增生为特征的疾病，影响到骨、骨髓、肾脏和神经系统。绝大多数患者只是某一克隆B细胞发生性变,出现一种抗体活性不明的异常免疫球蛋白，即骨髓球蛋白。 免疫缺陷 机体由于免疫系统的某种原因而降低或失去正常的免疫功能。免疫缺陷可以是原发的（由遗传或先天性原因引起），也可以是获得的（由其他疾病引起），免疫缺陷还可以分为体液免疫缺陷和细胞免疫缺陷两类。 原发性免疫缺陷有遗传性的，如性联锁或常染色体遗传；有先天性的，如胚胎发育不良，造成某一组织成分缺失或酶系统异常。常见的类型有①免疫球蛋白(Ig)缺陷：有些患者是各类Ig均缺乏或低下，也有些患者只是某一类或几类Ig缺损，而其他正常。其中最常见的是选择性IgA缺乏，每500～700人中就有一个患者,常见的症状是反复发生严重细菌感染。Ig缺陷的主要原因是B细胞系统异常，但也有些情况是抑制性T细胞功能亢进,使B细胞功能受到抑制;②细胞免疫缺陷：胸腺发育异常引起的疾病。典型实例是迪乔治综合症和内泽洛夫综合症，前者是由于先天性胸腺不发育引起的，后者是由于胸腺发育不全引起的。主要表现为多发性细胞内寄生的细菌和真菌感染以及病毒感染，迪乔治综合症还伴有甲状旁腺和心血管系统异常；③联合免疫缺陷：即缺乏体液免疫，又缺乏细胞免疫。 防治 除适当选用抗生素治疗有关感染外，对体液免疫缺陷应补充Ig，对细胞免疫缺陷可移植胎儿胸腺和胎肝以便提供干细胞，或注射胸腺素，也可考虑骨髓移植。
