单侧性索状卵巢综合征单侧性索状卵巢综合征 病名。亦称Slotnick-Goldfarb综合征、46,XX索状卵巢综合征。指患者具有正常女性体态,子宫和输卵管形态大致正常,但临床表现月经稀少到继发性闭经,左侧卵巢呈纤维索状,右侧卵巢发育不良的一种综合征。其病因机制尚不清楚,可能因为未被发现染色体丢失的结果。临床可见患者女性体型,乳房发育、皮肤、毛发生长和分布均正常;可有月经初潮,但经量较少,渐趋月经稀少到继发闭经,部分病人可伴有过早的卵巢功能早衰和过早绝经的内分泌症状;基础体温呈单相型,原发不孕;妇科检查可见外生殖器正常,阴道长度及子宫大小亦正常;腹腔镜及染色输卵管镜检查可见右侧卵巢发育不良,无排卵征象,左侧卵巢呈纤维索状;实验室检查LH和FSH稍高,GnRH兴奋试验提示卵巢衰竭,17-β雌二醇值很低,使用人类绝经期促性腺激素150U连续肌注3天,雌二醇值无变化,染色体核型为46,XX,分带正常;子宫内膜诊刮病理检查提示内膜萎缩,呈无功能性腺体和间质。本征尚无特殊治疗方法,可用雌-孕激素序贯法,能帮助维持第二性征,并可使月经来潮,对卵巢功能早衰和过早绝经而引起的自主神经紊乱症状有一定治疗作用。
