热带崩蚀性溃疡 一种急性特异性皮肤和皮下组织感染后形成的溃疡。又名热带溃疡或樊尚氏溃疡。发生部位在膝关节以下，很容易变成慢性。病因尚不清楚，溃疡分泌物培养有多种细菌,主要为樊尚氏螺旋体(Borrelia vincenti)和厌氧的拟杆菌。营养不良（尤其是 B族维生素缺乏）、虚弱、慢性病、皮肤受汗水浸泡等都是诱因。 本病多发生于热带潮湿地区，亚热带有散发病例。常在社会经济条件差、营养不良的劳动人群中发病，调查表明进食足够的动物蛋白质者很少发病。在监狱内生活条件差的囚犯中或丧失体育活动能力的人群中常有小流行。 起病前常见一侧踝关节附近的轻度外伤或虫咬伤，伤处曾作一般处理或曾用口吮吸，以后局部出现红色小丘疹或水疱，并出现发热、局部疼痛和全身不适。水疱破后形成小溃疡，溃疡很快向深部发展，波及肌肉筋膜甚至骨膜或骨质。数天内溃疡直径可达1～40cm,边缘隆起，中心凹下如碟状，表面覆以灰色或灰绿色伪膜及污秽的坏死组织，有粪臭。取溃疡组织作病理切片，显微镜下可见表层为凝固性坏死组织，其间有大量细菌，中间层是肉芽组织，底层为丰富的血管层，周围上皮样组织增生。溃疡处有疼痛感，局部腹股沟淋巴结肿大。由于病人的基础营养情况较差，皮损愈合很慢，50％病例在3个月内愈合,80％可迁延达半年。一旦形成慢性溃疡则不感疼痛，亦无臭味,少数(9％)慢性溃疡经数月至数年后发展成鳞状上皮细胞癌。急性期可根据溃疡的表现和臭味作出诊断，用溃疡深处分泌物涂片作革兰氏染色或用暗视野显微镜找螺旋体,可以明确病原体,也可作细菌培养。可借病理活检与雅司和鳞状上皮癌鉴别。对溃疡发展过程的详细描述对诊断也有帮助。 急性期要卧床，抬高患肢，加强营养，伤口用消毒生理盐水冲洗，除去坏死组织，有恶臭的伤口用过氧化氢清洗。全身可肌肉注射青霉素G或口服甲硝唑,大面积的创面恢复期可考虑植皮。
