骨化醇α , 阿法骨化醇药物名称:骨化醇α英文名:Alfacalcidol别名:骨化醇α , 阿法骨化醇 外文名:Alfacalcidol药理作用: 本品为维生素D3的一种较重要的活性代谢物,并有调节骨的无机盐的作用,其稳定性与维生素D3相同,在活体研究中证实它的疗效是1α,25-羟基维生素D3的一半。有促进肠道吸收钙,升高血清钙水平的作用,并有促进骨形成的作用。本品口服后由肠道迅速吸收进入血液,经肝脏微粒体25-羟基化酶作用使侧链25位羟基化后生成具有活性的1α,25-(OH)2D3,分布于肠道及骨等靶组织内与受体结合而表现出促进肠道吸收钙的作用、骨盐溶解作用以及骨形成作用等一系列生理活性。缺乏维生素D的大鼠及摘除肾的大鼠给予本品进行试验,可见促进肠道吸收钙、升高血清钙水平的作用。本品具有促进骨形成作用。从9d的鸡蛋中取出胚胎进行组织培养研究,证实正常的骨形成必须有1α,25-(OH)2D3。药代动力: 本品经小肠吸收,在肝脏中迅速代谢成机体内的天然活性物质1α,25-(OH)2D3。正常人口服4μg/d,给药后8~24h血中浓度达峰值,半衰期为2~4d。 适应症: 本品改善慢性肾功能不全、甲状旁腺机能减退、抗维生素D佝偻病、骨软化病时维生素D代谢异常产生的各种症状,如低钙血症、手足搐搦、骨痛、骨病变及骨质疏松症等。 用法用量:慢性肾功能不全、骨质疏松症:通常成人1次口服0.5~1.0μg/d;甲状旁腺机能减退,其他维生素D代谢异常病人口服1.0~4.0μg/d。根据疾病、年龄、症状、病情酌情增减。 不良反应:消化系:偶见食欲不振、恶心、嗳气、呕吐、胃部不适、腹部膨胀、消化不良、腹泻、便秘等;②精神神经系统:偶见头痛、头重、失眠、焦躁不安、四肢无力、倦怠,罕见目眩、困倦、胸痛、背痛、麻木、肩膀酸痛、老年性耳聋、耳鸣、记忆力减退等;③循环系统:偶见血压轻度上升,BUN、肌酸、酐升高(肾功能减退),罕见心悸;④皮肤:偶见瘙痒、皮疹,罕见热感;⑤虎:偶见结膜充血;⑥骨:偶见关节周围钙化;⑦其他:突起见肾结石,声音嘶哑。观察1785例,其中慢性肾功能不全1266例,出现不良反应者80例(6.32%)。主要有瘙痒、食 注意事项: 为防止给药过量,本品服用期间应定期测定血清钙值,应调整剂量使血清钙不超出正常值。出现高钙血症时应即停药。停药后血清钙达到正常范围,再开始减量用药。孕妇给药的安全性尚未确立,因此孕妇和可能怀孕的妇女应权衡利弊慎重用药。与含包产到户的药物并用时,可引起高镁血症,应慎重。高磷血症病人给药时,应并用磷酸结合药(氢氧化铝凝胶),降低血清磷值。 规格:0.25μg、0.5μg 、1.0μg 、3μg ;0.5μg/ml。 类别:抗骨质疏松药
