棘球蚴病棘球蚴病echinococcosis由棘球绦虫属的幼虫(棘球蚴,俗称包虫)寄生人体脏器而引起的疾病。又称包虫病。中国常见的有两种。①细粒棘球蚴病(囊型包虫病)。最常见的棘球蚴病,由细粒棘球绦虫的幼虫引起。成虫寄生在犬的小肠内,其虫卵为中间宿主牛、羊或人吞食后,六钩蚴在小肠内脱出,钻入肠壁随血流至肝脏或肺、肾、脑及骨骼内发育为棘球蚴,但发育缓慢,常在感染多年后才出现症状,主要为压迫局部组织或邻近器官而出现的症状。皮试及血清免疫学检查以及B超对诊断很有价值。治疗以手术为主,术前可辅以化疗,无法手术者可以长疗程丙硫苯咪唑治疗。②多房棘球蚴病(泡型棘球蚴病)。由多房棘球绦虫的幼虫引起。成虫寄生在狐、狼及犬的小肠内,中间宿主为鼠类,人并非其最适宜宿主,其囊蚴发育较慢,且无或甚少头节。它与细粒棘球蚴的不同在于生发层外无囊壁,以芽孢样向外突出,产生多个新囊泡,并向周围肝实质浸润,也可转移到其他器官,肝脏以外的病变多为转移灶。患者大多有打猎或与皮毛接触史。临床上有肝脏肿大、表面呈结节状、质硬等症状,易误诊为原发性肝癌。皮试及血清免疫学检查对诊断具有重要价值。治疗以早期手术切除病变为主,也曾有以丙硫苯咪唑治愈的病例报告。
