尘肺尘肺pneumoconiosis长期吸入粉尘所致的以肺组织纤维性病变为主的疾病。也有人认为是粉尘在肺内的蓄积和引起的组织反应。中国及世界大多数国家政府规定其为法定职业病。可能发生尘肺的主要工种有各种矿山的掘进工、风钻工、采矿工、爆破工、支柱工、搬运工等;耐火材料、玻璃、陶瓷、建筑材料工业的粉碎工及筛粉工、配粉工、搬运工、包装工等,以及其他生产过程中接触各种粉尘的工人。吸入大量粉尘,人体呼吸器官的防御功能不能将其过滤、附着、阻留,或粉尘沉积于肺泡又不能完成清除时可引起尘肺。因此,保持呼吸器官的良好自御功能有重要的意义。因粉尘的致病性及其浓度不同,从开始接尘到发现临床尘肺,一般要十数年或更长的时间。初期尘肺常在进行X射线胸片检查时才被发现,此时患者可无明显临床症状。病程进展或有合并症时,可出现气短、胸闷、胸痛、呼吸困难、咳嗽及咯痰等症状以及心肺功能、化验指标等的改变。诊断主要依据确切可靠的粉尘作业史及质量良好的胸部X射线片上符合尘肺诊断标准的改变,并排除其他疾病的可能。在中国诊断尘肺由各级卫生主管部门授权的尘肺诊断小组进行。目前尚无满意的特异疗法,尘肺诊断后应及时调离粉尘作业,采取积极的对症及恢复功能的综合治疗。预防的根本办法是做好防尘和降尘工作,使作业环境空气中粉尘浓度符合生产环境空气粉尘卫生标准的要求。
