军团菌感染军团菌感染legionellosis由军团杆菌所致的感染。可散发成局部流行。军团杆菌系需氧革兰氏阴性杆菌,以嗜肺军团菌最易致病。临床有两种类型:①庞蒂亚克热。为轻型,症状似感冒,无肺炎,多在短期内恢复。②军团菌肺炎。病情较重,病初有乏力、肌痛、干咳和发热,1~2天后症状加重,有高热、反复寒战、胸痛、相对缓脉等。肺部可有湿啰音和实变体征,可有胃肠道症状和肝肾功能异常,严重者可伴休克、呼吸困难及神志不清等,死亡率较高。军团菌肺炎与其他细菌性肺炎从临床上难以鉴别,确诊依靠血清抗军团菌抗体的检测(恢复期效价较急性期呈4倍增长),或从痰等分泌物中培养分离出病原菌。尿中检测出军团菌抗原可作早期诊断。此外亦可用直接免疫荧光法检测病原菌。治疗首选红霉素,疗程3周左右,疗效不佳者加用利福平。军团菌
