泌尿系统结石泌尿系统结石urinarysystem,lithiasisin发生于泌尿系统的结石。又称尿石症。包括肾、输尿管、膀胱和尿道的结石。尿石症的发病率有明显的地区差异,世界上有许多尿石高发地区,中国南方的发病率远高于北方。广东东莞发病率高达1.6‰,实属罕见。尿石症的发生发展与营养状况有密切关系,贫穷落后国家食物中以植物蛋白为主,尿中缺乏磷酸盐,容易发生膀胱结石,小儿尤为常见,而发达国家常见成人含钙肾结石。尿石形成的学说很多,如成核学说、基质学说、结晶抑制物学说,还没有任何学说能说明全部尿石的形成机制。泌尿系统的梗阻、异物和感染可促进尿石形成,反之,尿石又可以是梗阻、感染的原因。代谢性疾病如甲状旁腺功能亢进、痛风、草酸以及胱氨酸等代谢异常也可以是尿石形成的原因。发生在泌尿系统内的结石标本尿石在泌尿系统管腔内可以刺激粘膜引起肌肉痉挛,肾输尿管结石引起肾绞痛,膀胱部位结石在排尿时剧烈疼痛。结石堵塞管道,不仅可使疼痛加剧,且可影响尿液排出,引起肾输尿管积水,甚至肾功能不全。肾输尿管结石常表现为发作性肾绞痛,伴恶心呕吐,疼痛向下腹部、大腿内侧放射,肾区有叩击痛。膀胱结石可有排尿时尿线中断,并觉尿道剧痛。尿石症在疼痛发作时均伴血尿,一般血量不多或仅有镜下血尿。双侧肾输尿管结石可能引起无尿,长期梗阻可导致尿毒症。必须照全泌尿系统平片,包括肾、输尿管、膀胱,必要时包括尿道,可以发现结石钙化影,但结石过小或以尿酸成分为主时也可不显影。尿石症检查必须注意双肾功能情况即行排泄性泌尿系造影,并了解有无代谢异常疾病。泌尿系统结石尿石小于5毫米时,一般可能自然排出,服中药如海金砂、金钱草、滑石、蓄等,有助于排出,甚至可排出近1厘米大小结石。较大尿石或已引起梗阻时,应进行积极治疗,体外冲击波碎石、经皮肾镜和输尿管镜碎石等各种先进治疗方法已代替了以前的手术治疗,目前开放手术治疗仅适用于上述方法治疗有困难的病例。对有代谢疾病的尿石症病例应进行病因治疗。多饮水,保持足够的尿量可以减少尿石形成和复发。草酸钙结石应避免吃菠菜等高草酸食物以及牛奶等含钙高的食物。高尿酸血症病人需服枸橼酸钾等以碱化尿液,提高尿酸的溶解度,少吃啤酒、动物内脏、肉类等。
