职业危害因素职业危害因素occupational harzard zhiye we一hai yinsu职业危害因素(oeeupational hazard)在生产劳动过程中,因生产需要或伴随生产而构成直接或间接危害劳动者健康的成分,统称为生产性有害因素,或称为职业危害因素。根据其性质主要分为物理性、化学性、生物性三种职业危害因素。 物理性有害因素(l)噪声、超声及次声:强烈噪声可造成急性鼓膜损伤,长期慢性作用可使听力降低乃至耳聋。在超声波的直接作用下,可出现指端坏死。(2)振动:如长期使用凿岩机、链锯、油锯等机器操作时,可出现手指和前腕部振动病症状。(3)电磁辐射:红外线辐射可产生皮肤灼伤,在加热炉看火的工人中偶可造成视网膜灼伤或白内障。强紫外线的直接照射,可使电焊工发生电光性眼炎;接触沥青的工人在紫外线的同时作用下,可促使光感性皮炎的发生。在高频及微波辐射作用下,可出现神经衰弱综合征。激光可引起视网膜灼伤、眼炎及皮炎。(4)电离辐射(X线、丫线等):所致危害(见急性放射病、慢性放射病、局部皮肤放射性损伤)。(5)异常的气象条件:如高气温、高气湿、强热辐射及低温等。(6)异常气压:高气压可发生潜函病(或称减压病);低气压环境可发生航空病、高原病。 化学性有害因素(l)工业毒物:常见的有金属类(如铅、锰、汞等),非金属类(如砷、磷等),有机溶剂(如苯、甲苯、二甲苯、汽油、甲醇、二硫化碳等),高分子化合物(如有机氟、丙烯睛等),苯的硝基和氨基化合物(如三硝基甲苯、苯胺等),窒息性或刺激性气体(如一氧化碳、硫化氢、氰化氢、氯、二氧化硫等)引起的职业中毒。(2)粉尘:如含游离二氧化硅较高的粉尘引起的矽肺,硅酸盐中石棉粉尘引起的石棉肺,滑石粉尘引起的滑石肺等,以及由有机粉尘和无机粉尘(如皮毛尘中的毛和二氧化硅等)引起的混合性尘肺(如皮毛工尘肺)。纯有机尘是否引起尘肺的问题,尚有争论。金属粉尘如锡等进人机体后,胸部X线片虽有改变,但主诉症状轻微,且脱离粉尘后经若干年,一度沉积于肺内的粉尘似尚有排出的可能(X线胸片有明显的好转)。(3)刺激性和致敏性物质:如某些树脂在加工时产生的热解产物呈气体或蒸气,多数为液沐如矿物油、大漆、煤焦油等。对少、类虽不产生明显的刺激现象(如可引起结膜炎、上呼吸道炎及深部呼吸道炎)或致敏(变态反应)现象(如过敏性皮炎、鼻炎、支气管哮喘等)。(4)致癌物:如接触联苯胺、月一苯胺、四氨基二苯、四硝基二苯等可产生泌尿系肿瘤;接触双氯甲醚、三氯甲苯可产生肺癌;接触石棉可产生肺癌或间皮瘤;接触苯可产生白血病等。(5)致遗传突变和致畸胎物;如接触苯、氯乙烯、氯丁二烯等的工人,其外周淋巴细胞的染色体畸变率增高。
