镉中毒镉中毒cadmiumpoisoning通过镉及其化合物经食物、水和空气进入人体后产生的毒害作用。有急性、慢性中毒之分。工业生产中吸入大量的氧化镉烟雾可发生急性中毒。早期表现为咽痛、咳嗽、胸闷、气短、头晕、恶心、全身酸痛、无力、发热等,严重时可出现中毒性肺水肿或化学性肺炎,中毒者高度呼吸困难,咯大量泡沫血色痰,可因急性呼吸衰竭而危及生命。用镀镉的器皿调制或存放酸性食物或饮料,食物和饮料中可含镉,误食后可引起中毒。潜伏期短,通常经10~20分钟后,即可发生恶心、呕吐、腹痛、腹泻等消化道症状。严重者可有眩晕、大汗、虚脱、四肢麻木、抽搐。长期接触镉及其化合物可产生慢性中毒,引起肾脏损害,主要表现为尿中含大量低分子量的蛋白,肾小球的滤过功能虽属正常,但肾小管的回吸收功能却减低,尿镉排出增加。吸入氧化镉烟雾所致的急性中毒,其治疗关键在于防止肺水肿。应及早撤离有毒现场,安静卧床,吸氧,保持呼吸道通畅,吸入消泡灵(二甲基硅油)以消除泡沫;肾上腺皮质激素能降低毛细血管通透性,应早期足量使用;限制液体入量,给予抗生素防止继发感染;急性食入性镉中毒主要进行对症治疗;给予大量补液,注射阿托品用来止吐和消除腹痛;慢性镉中毒引起肾脏损害者,膳食中应增加钙和磷酸盐的摄入,供给充足的锌和蛋白质。金属络合剂如依地酸二钠钙虽可增加镉的排出,但可加重肾脏的损害,故不宜使用。为预防镉中毒,熔炼和使用镉及其化合物的生产场所应具有良好的通风和密闭装置;不应在生产场所进食和吸烟;镀镉器皿不能用来存放食品,特别是醋类等酸性食品;从事镉作业的职工,应进行定期的健康检查;有明确肝肾损害和心血管疾病者不宜从事接触镉的工作。
