局部皮肤放射性损伤局部皮肤放射性损伤part derma radioactive harm JubuP一fu fangshex一ng sunshong局部皮肤放射性损伤(part derma radioae-tive harm)身体某部位皮肤一次或短时间(数日)内受到大剂量放射性物质照射,或长期受到放射性物质超容许剂量标准的照射所引起的身体某部位皮肤的伤害或称局部放射性皮炎。 临珠表现分为急性型和慢性型两类表现。 急性型局部皮肤一次或短时间(数日)内受到大于5一loGy(戈瑞)(500一l000rad(拉德))的照射,受照后约1一2周时间出现脱毛、红斑、毛囊疹、水疙、溃疡和坏死等急性放射性皮炎改变。 慢性型亦称为慢性放射性皮炎。由于局部长期受到超过容许剂量标准照射引起。多发生在长期从事X射线下整骨的医务人员及类似职业工作人员。受照后约数年出现皮肤粗糙、辍裂、角化过度、疵状突起等,进而有长期不愈的溃疡。手部照射时还出现指甲的改变。 处理原则 急性型基本按灼伤原则处理。尽早保护受照局部。创面治疗宜选用刺激性小、具有抗感染能力、又能改善局部血液循环、促进组织生长的药物。必要时予以手术治疗。 慢性型(1)宜早期诊断并改善其工作条件,减少局部受照剂量。如,戴防射线手套并局部对症治疗。(2)对经久不愈的溃疡或局部增生,应尽早手术治疗。并宜及早调离放射性工作,进行局部对症治疗。 (于佩蓉)
