焦炉工人肺癌焦炉工人肺癌lung cancer among coke oven workers J旧0 lu gongren fe旧i焦炉l人肺癌(lung eaneer among eokeoven workers)焦化厂焦炉作业工人在长期接触焦炉逸散物后发生的与其职业性接触有病因学联系在肺部发现的原发性恶性肿瘤。焦炉逸散物中含有大量的致癌性多环芳烃,可使焦炉作业人群的肺癌率(发病率或死亡率)显著高于一般人群(非暴露人群,标准人群或比较人群),肺癌率随接触量、接触工龄和开始接触后的年数增加而增高。这些流行病学现象表明焦炉作业人群的肺癌分布具有职业病的病因学特征。卫生部已将焦炉工人肺癌列入1988年1月1日实施的新订职业病管理条例。 焦炉工人职业性肺癌和一般人群的肺癌一样,发病机理尚不清楚。除了职业因素以外,在临床上和病理上都和一般肺癌没有肯定的区别。尽管有时某些组织学类型似乎占有优势,例如鳞状细胞癌,也显然不能用于职业性肺癌的诊断。肺癌的大体类型,中央型或周边型,以及肺癌赖以诊断的其它检查之所见,尚不能区别职业性肺癌和一般肺癌。因此,焦炉工人职业性肺癌诊断主要依靠病因学指标,主要有:能反映患者接触水平的工种、接触工龄、开始接触以来的年数三项指标。 工种在焦炉顶部和炉侧(焦炉中部和底部)各工作场所从事炼焦作业的工种和辅助工种均有患职业性肺癌的危险性。各工种所在工作场所空气中焦炉逸散物浓度可反映患者的接触程度。因此工种是确立焦炉作业工人肺癌诊断的主要指标之一。 接触工龄从事焦炉作业的实际工龄sa以上为可能患职业性肺癌的工龄。 开始接触后的年数开始从事炼焦作业及至诊断肺癌的间隔年数至少10a方有确立诊断职业性肺癌的可能。 以上三项病因学指标,在诊断焦炉工人职业性肺癌时不能缺少,缺一不可。不同年代、不同地区、不伺焦化厂的职业流行病学调查和环境测定的总评价可确定和更改指标和指标的内容。足够的调查数据可缩短或延长指标的年数。 (董德甫)
