志贺氏菌属 肠杆菌科的 1属。1898年,因日本细菌学家志贺洁首先发现而得名。本属是人类细菌痢疾的病原菌,通称痢疾杆菌。 革兰氏阴性,菌体大小(0.5~0.7)×(2~3)微米,无芽胞,无鞭毛,无荚膜,有的菌株有菌毛。需氧或兼性厌氧,能在普通培养基上生长。菌落光滑圆形,微凸,无色半透明,直径约 2毫米,在液体培养基中呈均匀混浊生长。分离培养常采用肠道选择鉴别培养基。DNA 中的G+C克分子含量为50~52%。分解葡萄糖,产酸不产气。除宋内氏志贺氏菌迟缓发酵乳糖外,均不发酵乳糖,吲哚反应不定,甲基红试验阳性,VP试验阴性,不能利用柠檬酸盐,不分解尿素,不产生硫化氢,不液化明胶,赖氨酸脱羧酶阴性。对酸、高温和化学消毒剂敏感,日光直接照射30分钟或60℃10分钟即被杀死,在冰冻中能存活约3个月。 本属菌均有菌体(O)抗原,按生化反应和菌体抗原构造的不同可分为 4群39型(包括亚型)。将志贺氏菌感染豚鼠的角膜,可产生特殊症状的角膜结膜炎。因此,豚鼠角膜试验常作为本菌属新菌型的鉴定指标之一。 志贺氏菌依赖其表面的菌毛粘附于回肠末端和结肠粘膜的上皮细胞上,继而穿入上皮细胞内生长繁殖,并向邻近细胞扩散。由于内毒素的作用,引起局部粘膜炎症、坏死和溃疡等症状。当内毒素被吸收入血时,可引起发热、白细胞数增加,少数病例还可引起中毒性休克或弥漫性血管内凝血。此外,痢疾志贺氏菌1型和部分 2型还能产生肠毒素,引起肠粘膜分泌增加,肠毒素对神经系统有亲和力。志贺氏菌引起的菌血症较少见。患者病后有一定免疫力,但免疫期短,也不稳固。可能与本属细菌血清型特别多、相互间无交叉免疫性有关。常用磺胺、氯霉素、黄连素等药物治疗,但易产生耐药性。减毒口服活菌苗正在研制中。
