胆囊胆囊gallbladder为一肌性膜性囊,大多数脊椎动物有胆囊,可以贮存和浓缩胆汁,并将胆汁排入十二指肠。各种动物的肝分泌胆汁速率不同,其胆囊浓缩胆汁能力也不同。形态与结构人的胆囊位于肝下部的胆囊窝内,梨形,分胆囊底、体、颈3部,胆囊颈和胆囊管内粘膜皱襞呈螺旋状突入腔内,形成螺旋瓣,在胆囊颈处有1袋状膨大,叫做哈特曼氏囊。人的胆管也分3部,即肝胆管、胆囊管、总胆管。三者排列成“丫”形。肝胆管通肝;胆囊管通胆囊;总胆管止于十二指肠壁,开口处有奥狄氏括约肌,它调节流入十二指肠的胆汁量。胆囊壁分为粘膜层、肌层和外膜3层。功能①贮存胆汁。胆囊不仅能贮存相当于它容积的量,由于它能浓缩胆汁,故能贮存更多的肝胆汁。肝胆汁进入胆囊的贮存活动主要在夜间。②浓缩胆汁。胆囊壁能主动吸收Na+、Cl-和HCO3-,水随之被动吸收。由于水被大量吸收,故遗留下来的胆盐、胆固醇、磷脂和胆色素等即被浓缩。此外,胆囊还有分泌功能,可分泌粘蛋白等。在胆囊内被浓缩的胆汁,叫做胆囊胆汁。③调节胆道系统内的压力。胆囊收缩或舒张可调节胆道系统内的压力。当奥迪氏括约肌紧张性升高时,胆囊舒张,肝胆汁转流入胆囊,但不会引起胆道内压明显上升;当胆囊收缩时,胆道内压逐渐升高,一旦胆道内压超过奥迪氏括约肌内压,胆汁就进入十二指肠,胆道内压也就回降。所以,胆道内压的高低,不仅与胆囊舒缩有关,还与奥迪氏括约肌紧张性高低有关。活动的调节胆囊活动受激素和神经的调节。①激素调节。脂肪性食物进入小肠,可刺激肠粘膜内的I内分泌细胞释放胆囊收缩素,它能促使胆囊收缩和奥迪氏括约肌紧张性降低,从而引起胆汁排放(见胃肠激素);注射血管活性肠肽能强烈地抑制基础情况下胆囊的紧张性,也抑制胆囊收缩素对胆囊的作用;孕酮可减弱胆囊平滑肌的紧张性,并使胆囊对水的吸收减少,而致胆囊容积增大。胰多肽使胆囊舒张,它的作用恰与胆囊收缩素相反;生长抑素也使胆囊舒张,这与它抑制胆囊收缩素释放有关。②神经调节。电刺激狗的颈部迷走神经可使胆囊胆汁排放,这是胆囊收缩所引起的。切断狗颈两侧迷走神经后,胆囊容积约增大一倍、胆管松弛、胆囊收缩素刺激胆囊收缩的效应加强,后一效应可能是取消了血管活性肠肽能纤维对胆囊抑制作用所致。一般来说,刺激交感神经引起胆囊舒张。
