多寐　　以精神疲倦、不分昼夜时时欲睡、呼之能醒、醒后又想睡为主要临床特征的病证。又称嗜睡、多卧、嗜卧。多系阳虚阴盛、气血虚损、湿邪困脾所致。《内经》认为睡眠现象和阴阳二气有关，清醒是阳气盛的表现，入睡是阴气盛的结果，多寐的病机是阳虚阴盛。金代李东垣观察到多寐常见于脾胃虚弱的患者。清代沈金鳌《杂病源流犀烛》认为多寐属心脾两虚。清代何梦瑶《医碥》则认为多寐可由热邪引起。西医的发作性睡病、神经官能症与多寐症状类似者，可参考中医对多寐的辨治。　　　　多寐的病因有阳气虚衰、脾胃气虚、湿邪困阻、瘀血阻窍、痰热内蕴的不同。阳气虚衰多见于禀赋不足，老年或久病的患者，脾胃气虚多由思虑劳倦所致。此外，久居湿地，感受外湿，或过食生冷瓜果，损伤脾胃，导致内湿；或久病血行不利，或外伤导致络脉瘀滞，均可引起多寐。在诸多病因中，以阳虚与湿困最为多见。阳虚则阴盛，故懈怠嗜卧；湿困则清阳不升，疲困多寐。　　　　多寐的辨治，关键在于区分虚、实、寒、热，辨别其受病的脏腑。常见证型有：①阳气虚衰。证见精神疲惫，终日嗜睡，懒于言语，畏寒肢冷，舌淡，苔薄，脉沉细无力。治宜益气温阳，常用附子理中汤。若兼见思维迟钝，记忆力减退，甚或健忘，属髓海不足，治宜益肾填精，偏肾阳虚者用右归丸，偏肾阴虚者用左归丸。②脾气不足。证见精神倦怠，嗜睡，食少，面色萎黄，四肢无力，舌苔薄白，脉虚弱。治宜益气升清，常用人参益气汤。③湿邪困脾。证见头重如裹，昏昏嗜睡，肢体沉重，胸闷泛恶，纳谷减少，大便稀溏，舌苔白腻，脉濡。治宜燥湿健脾，常用太乙神术散。④痰热内蕴。证见精神萎顿，形体肥胖，昼夜嗜睡，胸闷痰多，舌苔厚腻，脉滑。治宜化痰醒神，常用温胆汤。⑤瘀血阻窍。证见头昏头痛，神倦嗜睡，病程较久，或头部有外伤史，舌质紫黯或有瘀斑，脉涩。治宜化瘀通络，常用通窍活血汤。⑥胆经郁热。证见多寐，口苦，小便色黄，舌质偏红，苔少，脉弦数。用清热醒神法，可用生酸枣仁30克，研末，清茶调服。若证见虚实错杂，则当权衡其主证与兼证，确定治疗措施。如气虚夹痰，益气为主，兼以化痰；阳虚夹瘀，温阳为主，兼以化瘀；年高髓海不足，兼痰瘀阻窍，益肾填精为主，参入化瘀通络之品。　　　　多寐实证易治，虚证见效较慢。
