再生障碍性贫血再生障碍性贫血aplasticanemia骨髓造血组织显著减少而引起造血功能衰竭所产生的贫血。因骨髓造血干细胞除制造红细胞外，还制造粒细胞和血小板，所以该病常表现为全血细胞减少。该病在中国比较常见，以青壮年居多，男性多于女性，临床上有急性型和慢性型两种。多数病例原因不明，称为原发性再生障碍性贫血；部分找到原因者称为继发性再生障碍性贫血，可能的原因如下：①药物及化学物质如细胞毒类药物如烷化剂等，大剂量时任何人都可引起骨髓抑制；另一些药物如氯霉素、保泰松等，仅一般剂量或小剂量即可在某些人中引起该病，化学物质如苯等亦如此，这可能与个体特异性有关。②物理因素如X线和各种电离辐射。③感染如病毒性肝炎等。④可见于某些疾病如慢性肾衰等的晚期，此外还有先天性的（范可尼贫血）。发生机制有3种：①造血干细胞缺乏，干细胞具有自我更新能力和多向分化潜能，在相应调节因子作用下分化成各种血细胞，该病造血干细胞有质和量的缺陷。②骨髓微环境缺陷不适于造血干细胞的生长发育。③免疫调节异常，特别是Ts细胞增高抑制了干细胞的生长。该病临床呈进行性贫血，可伴皮肤粘膜出血和各种感染，血象和骨髓象检查可帮助确诊。急性型除临床发病急而重外，网织红细胞＜1％或＜1.5万／微升，白细胞明显减少，中性粒细胞＜500／微升，血小板＜2万／微升，骨髓多部位增生明显减低，非造血细胞增多；慢性型一般发病慢，症状相对较轻，血象呈三系减少和网织红细胞减低，但较急性型为高，骨髓至少一个部位增生不良，巨核细胞减少或消失，一般抗贫血治疗无效，在排除其他引起全血细胞减少的疾病后即可诊断。该病治疗显效很慢，常需数月至数年，除应去除病因外，主要治疗措施有：①对症治疗。输新鲜全血或成分血（根据贫血、出血和感染的不同，分别输注红细胞、血小板或白细胞）；控制感染；有出血者可加用皮质激素。②针对不同的发病机理给予相应的治疗。造血干细胞缺乏时可给雄性激素（丙酸睾丸酮或康力龙或大力补）；骨髓移植；胎肝细胞悬液或脐带血输注；近年来用白介素3和集落刺激因子等。骨髓微环境缺陷用654-2或一叶秋碱或硝酸士的宁。免疫调节异常时可用抗胸腺细胞或抗淋巴细胞球蛋白、皮质激素、左旋咪唑。③其他如脾切除和口服氯化钴部分有效。④中医中药也有效。
