经前期紧张综合征经前期紧张综合征premenstrual tension syndrome 病名。亦称经前期紧张症。中医称经行前后诸证。指生育期妇女周期性地于月经前7～14天开始，直至月经来潮的3～4日这一段时间，出现烦躁、易怒、悲伤、失眠、头痛、乳房胀痛、躯体疼痛、腹胀、浮肿、神经过敏等一系列神经精神症状，以及水盐代谢紊乱的一组综合征。本征病因尚不清楚。多数学者认为，可能与精神过度紧张，引起大脑皮质下神经功能紊乱，包括下丘脑对自主神经和垂体以及靶腺之间的调节失常有关。当抗利尿激素、肾上腺皮质激素的分泌增加时，则出现水钠潴留现象。亦可因雌激素代谢障碍和孕激素之间的比例失调，引起下丘脑和垂体间功能紊乱。近来有人认为，本征是由于精神刺激，使下丘脑多巴胺的含量减少，造成泌乳激素释放抑制因子的抑制作用减弱，导致垂体泌乳激素释放增加，产生乳房充血、胀痛、水肿，头痛、烦躁。精神过度紧张时亦可引起继发性的醛固酮分泌，产生水钠潴留。本征患者月经周期均正常，于月经来潮之前7～14日出现症状，并逐日加重，至经期达到高峰，行经开始，顿见缓解或消失，如此循环往复，呈周期性变化。临床上可出现精神神经症状，如精神紧张、神经过敏、易激动、倦怠、恐惧、失眠、忧郁、乏力、性欲异常、嗜睡、孤僻、头痛、思想不集中、易流泪、尿频感、严重者有迫害妄想及自杀的念头等；程度不同的水肿，乳房胀痛及触痛，恶心，呕吐，小便沉重感，个别有关节酸痛、荨麻 疹、痤疮、低热、面部潮热感，月经前口渴、多饮、贪食及特殊嗜食(如喜酸辣、香甜食品)以及低血糖倾向等表现。实验室检查，部分病例血、尿中雌激素浓度升高，或雌激素/孕酮比 率升高。阴道涂片呈卵泡期持久现象，即阴道上皮营养状况不正常，周期前半期，角化程度低于正常，月经中期，角化程度突然升高，可达80%～90%，甚至全部角化，说明雌激素过高。其治疗一般采用精神疗法、激素、药物等综合疗法。精神疗法主要是避免紧张、劳逸结合；激素治疗包括口服避孕药、孕酮、睾丸素和绒毛膜促性腺激素；药物包括利尿剂、维生素A、维生素B6、溴隐亭、碳酸锂、弱安定剂、三环抗忧郁剂；以及抗组胺药；综合治 疗包括精神疗法、卵巢X线照射、高蛋白饮食；中药采用中医辨证施治。
