雌激素与阿茨海默病雌激素与阿茨海默病 1907年,Alzheimer Alosis首次报道1例早老性痴呆患者,即“阿茨海默病”(Alzheimer’s disease,AD)。临床表现主要为痴呆,即记忆丧失、失语失认、定向理解、计算判断障碍,在性格、行为、情绪上均有改变。AD占所有痴呆症的50%以上。65岁以上的人群患病率为5%,之后年龄每增加5岁,患病率增加1倍。85岁以上人群,50%的人患此病。研究结果表明,男女两性患病率有明显差异,女性是男性的1.5~3倍。AD的病因尚不清楚。病理生理变化是弥漫性大脑皮质萎缩,大量神经细胞变性、丧失,以有重要认知功能的基底前脑和海马部皮层区胆碱能神经元损害最严重;神经细胞内、外β淀粉样蛋白沉积,形成神经原纤维缠结及老年斑、突触密度减低;合成乙酰胆碱所必须的胆碱乙酰基转移酶活性低下,谷氨酸盐、多巴胺、5-羟色胺、去甲肾上腺素能神经元也有累及。需根据脑部病理结果以明确诊断。目前尚缺乏根治的方法。胆碱酯酶抑制剂可能缓解部分轻中症患者的病程,维生素E、非甾体抗炎剂、糖皮质激素等也可能有效。近期的流行病学研究结果表明,雌激素替代治疗(ERT)可以降低妇女患AD的危险,推迟发病年龄,并有一定的剂量—效应关系。雌激素改善AD的可能机制是:①雌激素减低,可能对神经元的功能和生长产生直接影响,补充雌激素可逆转这一过程;②雌激素减低时,神经生长因子和营养因子的合成减少,补充雌激素可恢复其合成;③雌激素可减少β淀粉样物质在神经细胞中的沉积;④雌激素还可能通过调整炎症反应,调节氧化过程,改变脑部供血途径。
