针刺手法针刺手法在进针过程中，进针后至出针前对医针和所刺腧穴施行操作的方法。包括基本手法、辅助手法、补泻手法。基本手法主要有提插法和捻转法。①提插法。将针从浅层插向深层，再由深层提到浅层，如此反复上提下插。②捻转法。将针入一定深度后左右来回旋转的方法。辅助手法是针刺时，对针柄、针体和腧穴所在经脉进行的辅助动作，主要有以下几种：①循法。用手指顺着经脉的循行经路，在腧穴的上下部轻轻循按。主要是激发经气的运行、而使针刺容易得气。②弹法。用手指轻弹针尾，使针体微微震动，以加强针感。③刮法。用拇指抵住针尾，以食指或中指的指甲轻刮针柄；或拿食、中指抵住针尾，以拇指指甲轻刮针柄，或用拇、食两指从下向上轻刮针柄、称“旋刮”，可以加强针感和促使针感扩散。④摇法。轻轻摇动针体，可以行气，直立针身而摇，可以加强针感；卧倒针身而摇，往往可促使针感向一定方向传导。⑤飞法。以捻转为主，一般将针先作较大幅度的捻转，然后松手，拇、食指张开，一捻一放，反复数次，如飞鸟展翅之状，可以加强针感。⑥震颤法。持针作小幅度的快速颤动，以增强针感。补泻手法，一般包括单式补泻法、复式补泻法两类。其中单式补泻法有以下几种：①捻转补泻。针下得气后，捻转角度小，用手轻，频率慢，操作时间短者为补。先深后浅，轻插重提，提插幅度大，频率快，操作时间长为泻法。②提插补泻。针下得气后，先浅后深，重插轻提、提插幅度小，频率慢，操作时间短为补法。进针时疾速刺入、多捻转，徐徐出针为泻。③迎随补泻。进针时针尖随经脉循行去的方向刺入为补法。④呼吸补泻。病人呼气时进针，吸气时出针为补。吸气时进针，呼气时出针为泻。⑤开阖补泻。出针后迅速按揉针孔为补。出针时摇大针孔而不立即揉按为泻。平补平泻，进针得气后均匀地提插旋转后，即可出针。复式补泻法主要有烧山火、透天凉等。①烧山火。将针刺入腧穴应刺深度的上1／3（天部），得气后行紧按慢提（或用捻转）法九数；然后再将针刺入中1／3（人部），同上法操作；再将针刺入下1／3（地部），仍同上法操作，然后将针慢慢提至上1／3，继续行针、反复3次，即将针按至地部留针，在操作过程中可使患者产生温热感。多用治冷痹顽麻、虚寒性疾病等。②透天凉。将针刺入腧穴深度的下1／3（地部），得气后行紧提慢按（或捻转）法六数，再将针紧提至中1／3（人部），同上法操作；然后再将针紧提至上1／3（天部），仍同上法操作；再将针缓慢地按至下1／3，如此反复操作3次，将针紧提至上1／3，即可留针。操作过程中可产生凉感。多用于热痹，急性痈肿等实热性疾病。
