肛瘘肛瘘analfistula指肛管或直肠与肛周皮肤相通的感染性管道。一般有内外两个瘘口称为简单肛瘘，一个内口和两个以上外口者称为复杂肛瘘。临床所见90％为简单的肛外瘘，多数由内口感染所致肛周脓肿破溃或切开引流不愈合形成，少数为结核性或其他特异性病变所致。主要表现为肛周外瘘口反复闭合破溃，并有少量分泌物渗出。与直肠相通的高位瘘口可有气体或粪便排出。治疗的原则是必须将内口及瘘道连同外口一并切除；高位复杂的肛瘘采用挂线和手术相结合的方法可不损伤肛门括约肌的功能；久治不愈的肛瘘应考虑到是否存在溃疡性结肠炎、局限性肠炎、阿米巴病、结核等疾患的可能性。肛瘘癌变者少见，必要时应做组织活检以明确诊断。肛瘘中医认为，若患处红肿硬痛，疮口凸起，脓液稠厚，病体尚壮者，属实证；管道软陷，疮口凹进，脓液稀薄，病体虚弱者，为虚证。本病以外治为主，如挂线，手术切开或插药线等均可随症采用。内治为辅，实证宜清热利湿，用二妙散、萆藓渗湿汤加减；虚证宜养阴清热，用青蒿鳖甲汤加减。保持肛门清洁，正确及时地处理肛周脓肿是预防本病的有效方法。
