大肠癌 位于大肠的常见恶性肿瘤。大多发生于40岁以上,男性患者约为女性的两倍。根据肿瘤发生的部位,可分为直肠癌和结肠癌。前者早期无明显症状,或大便带血,以后逐渐出现大便频、黏液便、便血、里急后重等症状。晚期因肠腔狭窄,大便外形变细或扁平。后者主要表现为腹痛,先呈间歇性隐痛,后为持续性疼痛、绞痛,便溏,带脓血,便次增多。晚期肠癌均可见消瘦,乏力,并可扪及腹中包块。根据临床表现结合直肠指诊、X线检查、癌胚抗原检查、结肠镜检或取活组织作病理检查,可予确诊。中医古代无大肠癌病名。 大肠癌病因尚不明确。一般认为,可能与直肠和结肠内慢性炎症刺激有关。中医学认为,此病多因忧思郁怒,或饮食不节导致气机不畅、运化失常,以至瘀血湿浊积聚而成。 肠癌的早、中期以根治手术治疗为主,术后配合中医药治疗以巩固疗效。晚期无手术指征的患者,以中医药治疗为主,配合化疗或姑息性放疗。中医辨证治疗根据患者的不同临床表现一般分为湿热瘀毒、气滞血瘀、脾肾寒湿、肝肾阴虚、气血两虚等证型辨证论治。①湿热瘀毒。证见腹痛腹胀,大便次数增多,夹带黏液脓血,或里急后重,或便溏便细,得矢气则胀减,腹有包块,舌黯红,或有瘀斑,苔黄,脉弦数。治宜清热利湿、解毒消积。方用白头翁汤合地榆槐角汤加减。②气滞血瘀。证见腹部刺痛,包块坚硬不移,腹胀,腹泻,里急后重,泻出紫黑脓血,舌紫黯或有瘀斑,苔黄,脉涩。治宜理气化滞、活血化瘀。方用膈下逐瘀汤加减。③脾肾寒湿。证见久泻久痢,形体消瘦,面色苍白,肠鸣腹泻,腹中绵绵作痛,喜按喜温,恶寒肢冷,或大便失禁,污浊频出,舌淡,苔白,脉沉细无力。治宜健脾温中,方用附子理中汤合补中益气汤加减。④肝肾阴虚。证见五心烦热,形体瘦削,头晕目眩,潮热盗汗,口渴咽干,大便不爽或燥结,少腹包块,腹痛便血,舌红或黯红,少苔或光剥,脉弦细而数。治宜滋补肝肾、清热解毒,方用知柏地黄汤合大补阴丸加减。⑤气血两虚。证见形体消瘦,神疲气短,头晕心悸,双目昏暗,纳差少食,大便溏薄,舌淡苔白或少苔,脉细弱。治宜补气养血,方用八珍汤加味。 大肠癌多在辨证论治的基础上配合使用专方专药。常用的专方有上海的大肠癌方、消癌丸、山甲苦参汤、清肠消肿汤、肠癌饼、肠癌灌肠剂等。常用于肠癌的中草药有半枝莲、喜树根、鸦胆子、败酱草、白花蛇舌草、红藤、蚤休、肿节风、石榴皮、蛇莓、儿茶、大黄、大蒜、地榆、白头翁、蟾蜍、地鳖虫、守宫等。
