胎盘部位滋养细胞肿瘤胎盘部位滋养细胞肿瘤是滋养细胞肿瘤中罕见的一种。由于其少见，常被误诊为绒毛膜癌、子宫平滑肌肉瘤(leiomyosarcoma)、子宫肉瘤(uterine sarcoma)、混合中胚窦瘤(mixed mesodermal tumour)、恶性间叶瘤(malignant mesenohymal tumor)等。也曾命名为绒毛膜瘤(chonoma)，合体细胞瘤(syncytioma)，不典型绒毛膜癌(atypical choriocarcinoma)、绒毛膜上皮病(chorionepitheliosis)以及滋养细胞假瘤(trophoblastie preudo tumor)。至1981年才确定统一用现名。这类肿瘤多数继发于足月产和流产，少见于葡萄胎之后。病变主要发生在胎盘部位肌壁。多数呈包块状局限于肌层或突出于宫腔，界限清楚。镜检最明显特点为单一形态的瘤细胞以单个、条索状、岛状或成片侵入子宫内膜及肌层。一般达浅肌层，但亦有侵入深肌层而达浆膜表面，甚至穿孔者。瘤细胞形态介于细胞滋养细胞(cytotrophoblast)和合体滋养细胞(syncytiotrophoblast)之间，但兼具两者功能，故命名为中间型滋养细胞(intermediatetrophoblast)。组织化学染色仅见局灶性细胞中HCG反应阳性。但HPL染色大部分阳性。临床主要表现为产后闭经，随之不规则出血。检查子宫增大。血HCG检查仅见低或中度升高，偶亦见阴性者。明确诊断一般较困难，需和稽留流产，绒毛膜癌，合体细胞子宫内膜炎以及其他肿瘤相区别。需要时可行刮宫、刮出物病检有助诊断，但需临床结合病理。这类肿瘤多半为良性，但亦有恶性，发生转移而死亡。良性者经切除子宫可以治愈，对化疗不太敏感，化疗只适用于转移病人。
