非甾体抗炎药非甾体抗炎药non-steroidalantiinflammatorydrugs具有抗炎、抗风湿作用,但在化学结构上又与抗炎、抗风湿的肾上腺皮质激素(甾体)迥然不同的一类药物。因具有解热止痛作用,故曾归属解热镇痛药。这类药物的不良反应比甾体药物小。作用主要有以下几种:①解热。外来刺激引起内热原,进一步导致前列腺素(PG)释放而致体生温升高,本类药物抑制PG合成酶,故可解热。②镇痛。PG是痛觉致敏物质,本类药物抑制PG释放而镇痛,一般对钝痛有效,但无成瘾性。③抗炎、抗风湿。本类药物抑制PG合成,使缓激肽等炎性物质的释放受阻,故有抗炎、抗风湿作用。炎性疼痛的止痛以吲哚美辛(消炎痛)的效果显著,次为阿司匹林。抗炎、抗风湿则以阿司匹林、氨基比林为较明显,次为吲哚美辛。又以阿司匹林的不良反应较少,故为抗风湿的首选药,但用量需较大。④抗血小板聚集(抗血栓)。阿斯匹林通过抑制环氧化酶而对血小板聚集有强大的、不可逆的抑制作用。这一作用在阻止血栓形成中具有重大意义。解热镇痛药常用以下几种:①乙酰水杨酸(阿司匹林)。其五大作用为:解热、镇痛(用于头痛、神经痛、肌肉痛、牙痛及痛经等)、抗炎、抗风湿和抗血栓。每一作用的用量略有差异。年老体弱或体温在40℃以上之高热者,解热宜用小量,以免因大汗而虚脱。本品的主要不良反应是出血,胃及十二指肠溃疡患者、嗜酒者(胃粘膜有损伤)均应慎用,必须使用时,可采用肠溶片或薄膜片并控制用量。可致皮疹、血管神经性水肿。有哮喘史者慎用。制剂:肠溶片。②对乙酰氨基酚(扑热息痛)。有解热镇痛作用,解热作用与阿司匹林相似,而镇痛作用则较弱。用于感冒发热、关节痛、神经痛、偏头痛、拔牙后疼痛和术后疼痛等。可引起恶心、呕吐、出汗、腹痛等反应。长期大量使用可致严重肝损害。3岁以内儿童及新生儿,因肝、肾功能未发育成熟,应避免使用。制剂:片剂。抗炎镇痛药有解热、镇痛及抗炎作用,尤其以解除炎性疼痛较突出。常用的有吲哚美辛(消炎痛)、毗罗昔康(炎痛喜康)、甲氯灭酸、萘普生、布洛芬、酮基布洛芬(优洛芬)及芬布芬等。①吲哚美辛(消炎痛)。通过抑制PG合成酶而有解热、镇痛、抗炎作用,且作用极强。解热及缓解炎性疼痛的作用很明显,常用于急、慢性风湿性关节炎、痛风性关节炎及癌性疼痛,也可用于滑囊炎、腱鞘炎等;有抗血小板聚集作用,可防止血栓形成,但不如阿司匹林作用明显;治疗白色氏病,疗效颇佳;用于胆绞痛、输尿管结石所致的绞痛、偏头痛和月经痛等。内服常有消化道反应如恶心、呕吐、腹痛、腹泻及溃疡出血等;中枢神经系统反应有头晕、头痛等;可引起黄疸或转氨酶升高等肝损害;有粒细胞减少等血液系统反应;有皮疹、哮喘等过敏反应;溃疡病、震颤麻痹、精神病、癫痫、支气管哮喘患者及肾功能不全者及孕妇、儿童均应避免使用。制剂:胶囊剂和栓剂。②布洛芬。其抗炎、镇痛、解热作用比阿司匹林及扑热息痛均强。常用于风湿性关节炎、肩周炎及骨关节炎等症,适用于不能耐受阿司匹林者,对痛风性关节炎也有疗效。现已制成缓释长效制剂供用。偶有消化道不良反应及皮疹等,也能抑制血小板聚集,故可引起出血。溃疡病及有出血倾向者慎用。制剂:片剂。
