抗抑郁药抗抑郁药antidepressivedrugs主要用于治疗抑郁症及其他精神疾病伴发的抑郁情绪的药物。主要有单胺氧化酶抑制剂和三环类抗抑郁药两大类,新研制的四环类抗抑郁药的治疗作用和副作用,与三环类相似。单胺氧化酶抑制剂的副作用大,临床上已很少应用。三环类抗抑郁药包括阿米替林、多虑平、丙咪嗪和氯丙咪嗪等,为治疗抑郁症所首选,已基本替代用电痉挛治疗抑郁症。这类抗抑郁药口服后很快吸收,在肝内代谢,由尿排出,可治疗各种抑郁症,对内源性抑郁症疗效尤佳;对反应性抑郁症、抑郁性神经症、躯体疾病或抗精神病药物治疗伴发的抑郁症状也有疗效。另外,氯丙咪嗪还用来治疗强迫症。严重的心、肝、肾疾病和青光眼病人禁用,老年人和孕妇要慎用。各种三环类药物的抗抑郁效果相似,但是,阿米替林和多虑平具有镇静作用,适于伴有焦虑症状的抑郁症;丙咪嗪和氯丙咪嗪具有激活作用,适于伴有迟滞症状的抑郁症。治疗时要逐渐加量,4周左右显效。抑郁症状消失后仍须服治疗量一个月左右,然后逐渐减少,维持治疗6个月左右。药物的副作用是困倦、口干、视物模糊、便秘、心跳加快、排尿困难和体位性低血压,这类副作用一般不影响治疗,在治疗过程中可逐渐适应;严重的心血管副作用、尿潴留和肠麻痹少见。过量可致急性中毒甚至死亡。
