背景技术
:病理性近视(pathologicmyopia,pm)是指屈光度≤-6.0d,伴有眼轴延长,视网膜脉络膜退行性病变为特征的屈光不正。除了视力下降,还常伴有一些并发症如白内障、弱视、黄斑变性及视网膜脱离等等,是致盲的重要原因。据资料显示,特别是东亚国家包括中国大陆,香港及台湾地区等,近视的发病率近年来急剧增高,目前80-90%的学生患有近视,其中10-20%为高度近视。病理性近视在普通人群中发病率为l%,已成为失明的重要原因。预计到2020年近视人数将达到25亿,该病已日益成为严重的全球性公共卫生问题。近视的发病机制复杂,至今仍无法阐明。通过大量研究结果都肯定了遗传的作用,环境因素会起到调节修饰作用。在这种背景下,选择从遗传基因学角度进行研究寻找疾病原因,对于该病的防治意义重大。病理性近视的遗传方式表现出高度的异质性,不同的遗传模式都参与了疾病的发生。目前通过基因组扫描连锁、关联性分析和测序等方法已发现了大量的近视致病/易感基因区域,上述大量疾病连锁区域的发现为将来的基因克隆提供了线索,使中国人群的近视位点由2个增加到了10个。高度近视的治疗方法不容乐观。光学矫正配镜有助于增加视力却不能有效地控制近视的发展;手术控制眼轴延长及玻璃体手术激光治疗等,只可以暂缓病变进程,而且伴有一定的风险性与损伤性,远期效果也有待考证。因此,早诊断发现早干预,延缓病程发展对于保存病人视力是非常重要的。技术实现要素:本发明提出一种检测病理性近视的试剂盒及其使用方法和用途,该试剂盒灵敏度高,特异性强,只需要少量的dna样品即可测定突变位点是否变异,达到早期筛查的目的,有利于新药开发,具有良好的市场应用前景。本发明的一种检测病理性近视的试剂盒,其技术方案是这样实现的:所述试剂盒包括用于扩增kiaa1462基因第3外显子的第c.3385位基因位点的基因片段的试剂;所述试剂包括引物对,所述引物对的正义引物的序列为:tgggcgagatagcaactctt,所述引物对的反义引物的序列为:gaggttagcaacggaatgga。本发明的试剂盒是一种检测与病理性近视疾病有关的kiaa1462基因变异的试剂盒,包括引物对,通过引物对使dna样品进行pcr扩增,然后对扩增后的产物进行测序,从而得到kiaa1462基因第3外显子第c.3385位的基因位点是否发生突变,以判断是否是该类型病理性近视。本发明确定了kiaa1462基因突变是病理性高度近视的致病基因,采用本发明的试剂盒通过扩增和测序之后,如果kiaa1462基因第3外显子第c.3385位碱基的t突变为c,则确定为病理性近视;该突变型序列信息为:acaat/cagaa(-t-i/t-e-)异亮氨酸突变为苏氨酸。kiaa1462c.3385t>c突变与病理性近视的发病直接相关,检测是否突变可用于进行病理性近视的早期基因诊断及分型评估。本发明的检测突变位点序列用试剂盒,灵敏度高,特异性强,只需要少量dna样品就足以测定该突变位点是否变异,只检测一个基因位点即可获得是否是病理性近视的结果,达到早期筛查的目的,并有利于新药开发,具有良好的市场应用前景。作为一种优选的实施方案,所述试剂还包括缓冲液、混合液、氯化镁溶液、浓度为1u/μl的taq酶、浓度为30ng/μldna模板和二次蒸馏水,所述缓冲液是pcrbuffer,所述混合液为dntpmixture。本发明的试剂盒还配有pcrbuffer缓冲液、dntpmixture混合物、氯化镁溶液、taq酶、dna模板和二次蒸馏水,通常情况下,在25μl的试剂盒中各试剂的含量为:10×pcrbuffer1支,取5μl;dntpmixture中各组分为2ml,取5μl;2.5ml的氯化镁溶液取4μl;taq酶取2μl;10μl的正义引物取2μl,10μl的反义引物取2μl;dna模板取5μl;二次蒸馏水补足至25μl。这些pcrbuffer缓冲液、dntpmixture混合物、氯化镁溶液、taq酶、dna模板和二次蒸馏水优选是取自天根公司的pcrmix试剂盒。本发明的一种检测病理性近视的试剂盒的使用方法,其技术方案是这样实现的:包括以下步骤:1)取试剂盒,采用正义引物和反义引物对dna样品进行pcr扩增,得pcr产物;2)取步骤1)所得的pcr产物,加入pcr纯化试剂,进行纯化反应,纯化反应的条件是:37℃,20min,75℃,15min,12℃,保温,得纯化产物;3)以步骤2)所得的纯化产物为模板,进行测序处理,并测序。本发明的试剂盒在使用过程中,先采用正义引物和反义引物对dna样品进行pcr扩增,经过纯化之后,再进行测序处理,对处理后的产物进行测序分析,即可得到kiaa1462基因第3外显子第c.3385位的基因位点是否发生了突变,从而判断其是否为病理性近视。本发明的用试剂盒使用方便,操作简单,只需要少量dna样品就足以测定该突变位点是否变异,只检测一个基因位点即可获得是否是病理性近视的结果,灵敏度高,特异性强,达到早期筛查的目的,并有利于新药开发,具有良好的市场应用前景。本发明的dna样品来自的生物样本为人体体液或组织,包括但不限于血液、羊水、各种组织等;从样本中提取纯化dna,进行聚合酶链扩增反应,获取产物,使用引物对对产物进行测序反应,寻找样本中是否含有kiaa1462基因的突变。通常dna样品是来自外周血基因组dna,溶解于te中保存,dna样品的浓度通常为30ng/μl,置于-20℃的冰箱中保存。作为一种优选的实施方案,所述步骤1)中,pcr扩增的条件是:95℃,5min;95℃,30s,65℃,45s,72℃,45s,35个循环;72℃,10min;4℃,保温。本发明的pcr扩增的条件是现有的常规操作条件,操作方便,控制容易,扩增效果好,容易掌握。作为一种优选的实施方案,所述步骤2)中,pcr纯化试剂为exosap-it纯化试剂,所述纯化试剂的添加体积与pcr产物的体积之比为1:5。本发明采用sanger测序方法,优选使用usb公司的exosap-it纯化试剂即uspcr纯化试剂,从而提高纯化效果。本发明的一种检测病理性近视的试剂盒的用途,其技术方案是这样实现的:所述试剂盒用于检测病理性近视疾病。本发明的试剂盒可以用于检测病理性近视疾病,其灵敏度高,特异性强,使用方便,操作简单,达到早期筛查的目的,并有利于新药开发,具有良好的市场应用前景。与现有技术相比,本发明的有益效果是:本发明的试剂盒是一种检测与病理性近视疾病有关的kiaa1462基因变异的试剂盒,包括扩增引物对,通过引物对使dna样品进行pcr扩增,然后进行测序,从而得到kiaa1462基因第3外显子第c.3385位的基因位点是否发生突变,以判断是否是该类型病理性近视。本发明的检测突变位点序列用试剂盒,灵敏度高,特异性强,只需要少量dna样品就足以测定该突变位点是否变异,只检测一个基因位点即可获得是否是病理性近视的结果,使用方便,操作简单,达到了早期筛查的目的,并有利于新药开发,具有良好的市场应用前景。附图说明图1为本发明中五代病理性近视家系图;图2为本发明正常人的碱基序列图谱;图3为本发明病人一的碱基突变序列图谱;图4为本发明病人二的碱基突变序列图谱;图5为本发明病人三的碱基突变序列图谱;具体实施方式下面将结合本发明实施例中的附图,对本发明实施例中的技术方案进行清楚、完整地描述,显然,所描述的实施例仅仅是本发明一部分实施例,而不是全部的实施例。基于本发明中的实施例,本领域普通技术人员在没有作出创造性劳动前提下所获得的所有其他实施例,都属于本发明保护的范围。本发明的一种检测病理性近视的试剂盒,所述试剂盒包括用于扩增kiaa1462基因第3外显子的第c.3385位基因位点的基因片段的试剂;所述试剂包括引物对,所述引物对的正义引物的序列为:tgggcgagatagcaactctt,所述引物对的反义引物的序列为:gaggttagcaacggaatgga。优选地,所述试剂还包括缓冲液、混合液、氯化镁溶液、浓度为1u/μl的taq酶、浓度为30ng/μldna模板和二次蒸馏水,所述缓冲液是pcrbuffer,所述混合液为dntpmixture。本发明的一种检测病理性近视的试剂盒的使用方法,包括以下步骤:1)取试剂盒,采用正义引物和反义引物对dna样品进行pcr扩增,得pcr产物;2)取步骤1)所得的pcr产物,加入pcr纯化试剂,进行纯化反应,纯化反应的条件是:37℃,20min,75℃,15min,12℃,保温,得纯化产物;3)以步骤2)所得的纯化产物为模板,进行测序处理,并测序。优选地,所述步骤1)中,pcr扩增的条件是:95℃,5min;95℃,30s,65℃,45s,72℃,45s,35个循环;72℃,10min;4℃,保温。进一步地,所述步骤2)中,pcr纯化试剂为exosap-it纯化试剂,所述纯化试剂的添加体积与pcr产物的体积之比为1:5。本发明的一种检测病理性近视的试剂盒的用途,所述试剂盒用于检测病理性近视疾病。实施例一kiaa1462基因突变的检测一个常染色体显性遗传5代家系,严格遵照诊断标准:①近视度数≤-6.0d;②特征性的眼底改变包括视乳头弧形斑,豹纹眼底,黄斑变性及后巩膜葡萄肿等;③发病年龄在上小学之前;④近视度数进行性加深;⑤矫正视力<0.9;⑥排除非轴性病理性近视:如圆锥角膜、球形晶体等;⑦不伴有全身综合征;⑧只要一眼确诊病理性近视,即诊断为病理性近视患者;其中①②为必备的条件。确定表型后进行家系调查收集。利用cyrillic软件产生家系图谱。采集家系至少三位患者和一位正常直系亲属的外周血,提取基因组dna,使用agilenthumanallexonkit外显子组捕获芯片,制备cluster上机进行数据处理,然后经生物信息分析发现三名患者的全部变异数都在2-5千个左右,利用公共数据库进行过滤,并排除掉正常对照,得到的突变数为11个indel和3个snp。最后对家系内全部成员包括患者及正常亲属进行常规pcr扩增及sanger测序,检测14个变异区域,进行共分离验证,结果发现了所有患者携带着唯一的共分离位点,位于chr.10p11.23的exon3,kiaa1462c.3385t>c。本发明针对一个山东地区的常染色体显性遗传的病理性近视大家系见图1,经细胞遗传学检测及考虑单一性表型,基本排除了染色体致病的可能。经已知近视致病基因znf644,ccdc111,slc39a5,p4ha2和arr3等的突变筛查,未发现突变位点,结果提示该家系可能是新的致病基因突变。然后利用外显子测序技术寻找致病基因,发现了kiaa1462c.3385t>c,发生碱基突变,并且家系内所有患者表型呈共分离现象,随后经家系内正常成员和200名正常对照者测序该基因未发现此突变,从而确定kiaa1462基因突变是该病理性高度近视的致病基因。实施例二本发明的一种检测病理性近视的试剂盒,试剂盒包括用于扩增kiaa1462基因第3外显子的第c.3385位基因位点的基因片段的试剂;试剂包括引物对,引物对的正义引物的序列为:tgggcgagatagcaactctt,引物对的反义引物的序列为:gaggttagcaacggaatgga。该试剂还包括缓冲液、混合液、氯化镁溶液、浓度为1u/μl的taq酶、浓度为30ng/μldna模板和二次蒸馏水,缓冲液是pcrbuffer,混合液为dntpmixture。其中,试剂中各组分及用量如表1。表1试剂的组成序号组分用量110×pcrbuffer5μl2dntpmixture各2ml5μl32.5mlmgcl24μl4taq酶1u/μl2μl5正义引物10μl2μl6反义引物10μl2μl7dna模板30ng/μl5μl8ddh2o补足至25μl实施例三本发明的一种检测病理性近视的试剂盒的使用方法,包括以下步骤:1)基因组dna抽提:使用qiagen公司qiaampdnabloodminikit试剂盒,抽提外周血基因组dna;将所得dna溶解于te保存,dna工作浓度校正至3
